Housing Application Information

Shelter Care Ministries no longer takes applications for its housing programs.  All families who are homeless and interested in our  housing programs must contact the Single Point of Entry at 844-710-6919 or visit their office at 555 N. Court St., #301, Rockford, IL. Your application will be taken by this organization beginning Jan. 1, 2015.

Emergency Housing Program Overview  

Please see criteria and program requirements below.
Criteria

  • Short term shelter in private furnished apartment
  • Every family is assigned a Housing Advocate who assists with benefit acquisition, advocates on behalf of resident as needed, and supports the family as they work their plan.
  • Resident sets own agenda and goals; Housing Advocate holds resident accountable.
  • Adult members of the household meet with advocate at least once per week to work on a plan to end their episode of homelessness as quickly as possible.
  • Resident and agency both agree to a high standard of upkeep on the housing property.

Minimum Program Requirements

  • Family consists of adult(s) head of household with minor children in his/her custody
  • Family is currently homeless

Rapid Rehousing Program Overview -

  • Families self-select a rental property in the community. The lease is between the family and the property owner.
  • Agency pays deposit and first month's rent.
  • Families may qualify for up to nine months of rental assistance (on a graduated step-down plan) and up to twelve months of supportive services.
  • Each family is assigned a Housing Advocate who assists with selecting an affordable rental property, obtaining benefits  advocates on behalf of resident as needed and supports the family as they work their program plan.
  • Resident sets own agenda and goals; Housing Advocate holds resident accountable.
  • Adult members of the household meet with Advocate on average twice per month at program onset, which may decrease as family becomes more stable.
  • Resident and agency both agree to a high standard of upkeep on the housing property.

Minimum Program Requirements

  • Family consists of adult(s) head of household with minor children in his/her custody
  • Family is currently homeless
  • Family has a desire to rent privately in the community and agrees to create a plan to increase income to afford market-rate rent.

Permanent Supportive Housing Program

Three programs are available: Veteran Family Permanent Supportive Housing, Permanent Supportive Housing for Families who are chronically homeless and Disability Permanent Supportive Housing

All programs are designed for families who are currently homeless and have a head of household member with a chronic mental or physical disability.
Overview of Program

  • Long-term support to families experiencing homelessness and looking for the opportunity to stabilize their lives
  • Rent will be paid by Shelter Care.
  • Program participant pays their own utilities.
  • Participants agree to create a program plan, meet regularly with a Housing Advocate and be accountable for their choices.

Minimum Program Requirements

  • Family is literally homeless
  • Head of household has a disability
  • Adult(s) has minor children in his/her custody
